29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Production - On May 22, 2019, machine operators were exposed to machine hazards associated with moving parts when changing belts on the polishing machines. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform changing of belts [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Facility Wide - On April 17, 2018, the employer did not develop, document and utilize an energy control procedure (lockout/tagout program) and did not provide training on the energy control procedures to ensure that all required elements of the procedure were understood by employees and that the knowledge and skills required for the safe application, usage and removal of the energy controls were acquired by employees performing servicing or maintenance on a machine or an equipment such as polisher machines during service and maintenance such as changing belts on the polishing machines. 29 CFR 1910.95(c)(1): 29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8-hour time-weighted average (TWA) sound level of 85 dBA: On March 28, 2018, an effective hearing conservation program which included noise monitoring, audiometric testing of employees and training of employees as detailed in the standard was not instituted by Precision Chrome, Inc. a) An employee operating a chrome polishing machine was exposed to continuous noise levels at 83.6% of the allowable 8-hour, action level for noise (85 dBA). The equivalent dBA level of 83.6% is approximately 88.7 dBA. The sampling was performed on March 28, 2018 for 453 minutes. Zero exposure was assumed for the unsampled period of time, 27 minutes. b) An employee operating a chrome polishing machine was exposed to continuous noise levels at 67.2% of the allowable 8-hour, action level for noise (85 dBA). The equivalent dBA level of 67.2% is approximately 87.1 dBA. The sampling was performed on March 28, 2018 for 453 minutes. Zero exposure was assumed for the unsampled period of time, 27 minutes. c) An employee operating a steel polishing machine was exposed to continuous noise levels at 108.7% of the allowable 8-hour, action level for noise (85 dBA). The equivalent dBA level of 108.7% is approximately 90.6 dBA. The sampling was performed on March 28, 2018 for 454 minutes. Zero exposure was assumed for the unsampled period of time, 26 minutes. |
