Drive motor belts broke on horizontal impact crusher. They wrapped around the shive. The first few belts were tough to cut. The second to last belt cut very easy because the last belt broke while cutting the second to last belt. Cut the inside of left forearm with razor knife. 8 stitches.

No one on site witnessed the accident. It is presumed that EE fell off the ladder of the 777D EE was driving at the end of the shift. EE remembers nothing from the fall or the events that took place after. Said nothing about the fall when talking w/management, but complained of back pains and drove home. No one was aware of the fall until checking in with EE after hours.

Was walking on concrete and missed the step off and rolled right ankle. EE worked the rest of shift, no problems, was off 9/30/19 for personal day, came back 10/1/19. Sent to medical and put on light duty on 10/1/19. Came back to work and given return to full duty on 10/7/19. Will report on 4th Quarter injuries report. Did not miss work for ankle.

Two EEs were taking a fuel tank off a pavement roller. There was pressure in the pump that sprayed when the line was taken off. Both EEs were wearing safety glasses. The EE did not notice irritation until EE woke up the following day (Saturday). Sent EE to be checked Monday morning. Urgent care gave EE prescription eye drops. No lost work time.

The employee was filling a 777F with diesel fuel at the start of the shift. Strained right arm while doing so. The employee went back to full work hoping it would get better. We were informed on 10/5/2018 at the end of the shift. The employee wanted to see if it got better with rest. The employee asked to go to the doctor the following week. Physical therapy assigned 10/25/2018.

The EE was inside the man lift basket changing a grease line. The basket caught and slid off the steel frame causing a bouncing effect. Safety lanyard pulled EE back in basket. Bruised lip laceration, closed fracture of right side of maxilla and fracture of transverse process of spine w/o spinal cord lesion. ER said EE could return to work next day. See specialist. Sore body.

The employee bent over to pick up a metal chute door in tower 1 of the screening plant. When the employee came to me ee looked to be in pain and was having trouble moving. Took employee to an urgent care where the doctor prescribed a muscle relaxer at the employees request. Put on light duty and went back to work on the same day.

Mine EE annual hearing test came back over the acceptable level of 25db on 8/9/17. EE was re-tested on 8/25/17 per the regulations and again was over the 25db level. EE continues EE's enrollment in the Hearing Conservation Program (HCP) and all engineering & administrative controls are being reassessed.

Employee (EE) visited clinic on this 8/13 with complaint of stiff/sore neck resulting from work performed on 8/10. On 8/10 EE was working on Primary Crusher removing, cleaning, and replacing wear plates. EE was working in an awkward, stooped position. On 8/13 Dr. diagnosed neck sprain/back strain and ordered physical therapy. EE returned to work same day.

Employee (EE) was working inside primary crusher changing impact hammers. When witness struck the impact hammer with a sledge, a shard broke loose from the impact hammer and struck EE in the arm, lodging just below the skin. EE was transported by supervisor to local ER where Dr. cut out shard and applied 3 sutures. EE returned to work with no restrictions same day.

The truck landed on its' right rear corner in the dumped material the truck then fell forward causing the left front corner of the cab to striking the rock wall and sloughed or rolled back to the position. He seem lucid but bloody from cuts on his left arm and right hand and some cut on his head, was no visible bleeding at that time.
