29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, Items A-D of this section: a) On or about February 1, 2023, the employer failed to clearly identify all of the specific steps to be followed by employees to isolate and disable the 60" slitter, Yoder tension stand in order to safely de-energize the equipment and control the hazardous hydraulic energy. The control of hazardous energy procedures "Lock Out/Tag Out Document #: P406-B" and "Blade Selection and Set-Up Document #: P201-C" were developed for setup activities and did not contain specific steps to relieve hazardous hydraulic energy prior to equipment access. Additionally, specific verification steps were not prescribed to determine the effectiveness of the energy control measures. As a result, employees performing setup activities such as, but not limited to, carpet and pad change outs, had their hands and fingers exposed to caught-in crushing hazards associated with the specific machines unexpected clamp movement. The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1527613, citation number 1, item number 1-a and was affirmed as a final order on October 26, 2021, with respect to a workplace located at 951 Frontenac Rd, Naperville, IL 60563. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). |

On or about, February 1, 2023, the employer did not ensure that employees who performed service and maintenance tasks on the Yoder 60" slitter were adequately trained on the means and methods needed to effectively isolate and control hazardous energy sources. Employees were exposed to caught-in and amputation hazards associated with the unexpected closing of the machines tension stand and in-running nip points of rotating parts while performing service and maintenance activities, including, but not limited to, set-up, carpet and pad changes, and removing and installing guide rollers. 29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about February 1, 2023, the employer did not ensure that employees implemented energy control application steps prior to allowing them to perform service and maintenance activities on the tension stand of the Yoder manufactured 60" slitter. Employees had their hands and fingers exposed to caught-in and crushing hazards associated with the unexpected clamp closing while performing activities, such as, but not limited to inspecting, adjusting, and changing carpet. b) On or about February 1, 2023, the employer did not ensure that employees implemented energy control application steps prior to allowing them to perform service and maintenance activities on the slitting head (feed side) of the Yoder manufactured 60" slitter. Employees were exposed to caught-in and amputation hazards associated with in-running nip points and rotating parts while performing activities, such as, but not limited to, removing and installing guide rollers. As a result, the remaining applicable energy control elements, involving preparation for shutdown [1910.147(d)(1)], machine or equipment shutdown [1910.147(d)(2)], machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards, likely to result in serious injury. The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1527613, citation number 1, item number 1-b and was affirmed as a final order on October 26, 2021, with respect to a workplace located at 951 Frontenac Rd, Naperville, IL 60563. 29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following 29 CFR 1910.147(c)(4)(ii)(A)-(D): (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and, (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. 60 slitter, Yoder tension stand: on or about April 5, 2021, the control of hazardous energy procedures "Lock Out/Tag Out Document#: P406-A" and "Lock Out / Tag Out Procedures Hydraulic Unit - 60" Slitter Lockout Disconnect #5" were developed for preventive maintenance and repair work performed by maintenance personnel. These procedures did not contain the required specific information for use by non-maintenance personnel assigned to perform carpet inspections, adjustments, and changes (setup) which exposed their hands to a moving ram caught-in hazard between the upper and lower boards (clamp) when equipment was not de-energized in correct sequence to ensure safety. These procedures failed to clearly identify all of the specific steps to be followed by employees to isolate and disable the tension stand in order to safely de-energize the equipment and control the hazardous hydraulic energy. The procedure also lacked specific steps to relieve hazardous hydraulic energy prior to equipment access and specific verification steps were not prescribed to determine the effectiveness of the energy control measures. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). |

Because the equipment was not locked out, employees had their hands exposed to caught-in hazards from the unexpected clamp closing.
