Crossbraces shall not be used as a means of access. (a): On or about July 31, 2023, The employer did not ensure that supported scaffolds had a ladder or equivalent method for employees to access the upper work platforms. Employees climbed the crossbraces of the scaffold, exposing them to falls of approximately 10.8 feet to the ground below.

When the falling objects are too large, heavy or massive to be contained or deflected by any of the above-listed measures, the employer shall place such potential falling objects away from the edge of the surface from which they could fall and shall secure those materials as necessary to prevent their falling. (a): On or about July 31, 2023, The employer did not ensure that scaffolds had toeboards installed to contain or deflect tools or equipment from the upper level.
