Diversey Parkway, Chicago, Illinois: An employee was working in a trench excavation without adequate cave-in protection. The excavation measured 8 feet deep at the southwest corner, 7 feet at center, 6.6 feet at the northwest corner, 4.3 feet wide, and approximately 28 feet long. The employee was exposed to a cave-in hazard. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
