Apparent deficiencies in the items noted in 1926.1412(d)(1)(i) thru (xiv), at a minimum, were not observed: a. On or about December 3, 2013, at the above addressed jobsite, employees were exposed to a crushing hazard from falling loads while working with a National Series 400 mobile hydraulic crane. The employer failed to ensure that an adequate visual inspection of the equipment was made by a competent person before each shift. |
